DETAILS REGARDING THE COLORADO  CATHOLIC HEALTHCARE DIRECTIVE

This document is not intended to act as legal counsel or advice.  Please contact an attorney for any questions you might have regarding advance care planning.  On our website you will find 4 documents that constitute the Colorado Catholic Healthcare Directive.  The first document is a letter from the three bishops of Colorado.  The second document entitled The Catholic Context provides an explanation of the moral and faith aspects of end-of-life issues.  The third document - What is a Healthcare Directive? - explains what a medical durable power of attorney is and what it does.  The final document is the actual Medical Durable Power of Attorney form along with a statement of Catholic Beliefs regarding end-of-life care.  This form should be filled out and copies given to your designated agent and alternate agents and a copy should also be given to your doctor, while the original should remain in your possession.
